New Passport application

The application form is available for you to download. Please write clearly and complete the entire application.

New Passport Requirements

1. Completely filled-out form
2. Four Photographs (For Sudanese women. Need to wear Tarha).
3. Payment of national contribution tax according to applicants occupation.
4. Expired Sudanese passport (for passports that have completed 10 years from the date of issuance).
5. If the passport was lost or stolen, these documents are required:
  a) Original police report
  b) Copy of old passport
  c) Copy of Nationality Certificate.
6. Fees for students RM87.00; For non-students RM212.00
